Job Title: Nurse Coaching Lead Analyst ( Nurse Case Manager - RN)

Location: Arden, NC

Schedule:

Monday: Off
Tuesday: 8:30 AM – 4:30 PM
Wednesday: 6:30 AM – 2:30 PM
Thursday: 8:30 AM – 4:30 PM
Friday: 8:30 AM – 4:30 PM

Description:

We are seeking an experienced Nurse Coaching Lead Analyst to provide high-quality case management, telephone triage, and health coaching services. This role involves conducting comprehensive assessments and guiding members in making informed health care decisions. You will work independently with minimal supervision and use clinical judgment to deliver personalized care and education to patients.

Key Responsibilities:

Conduct telephone triage and symptom assessments using established protocols and clinical criteria
Provide members with appropriate medical guidance and education
Collaborate with healthcare professionals and interdisciplinary teams as needed
Use nursing experience, clinical software, and online tools to support decision-making
Lead or support complex Nurse Case Management projects
Offer mentorship and guidance to junior nursing staff
Monitor health trends and participate in process improvement within Nurse Coaching

Requirements:

Active and unrestricted Registered Nurse (RN) license
Strong clinical assessment and critical thinking skills
Ability to work independently with minimal supervision
Excellent communication and patient education abilities
